OWNER — OCCU�PIED REHABILITATION ASSISTANCE AGREEMENT <br /> FOR GENERAL CONTRACTORS <br /> HOME INVESTIUIENT PARTNERSHtPS PR�GRAM <br /> (Only if appiicable if assistance funded with HOME Funds) <br /> THiS AGREEMENT is entered into this 21 day of November 2013, by and between the Pasco County Board <br /> of County Commissioners (Community Development Division), (hereinafter referred to as the"County"}, and <br /> Covenant Communities. Inc. (hereinafter referred to as the"Contractor"). <br /> THIS AGREEMENT 1S ENTERED INTO BASED ON THE FOLLOWING FACTS: <br /> A. WHEREAS, Pasco County has been designateti by the United States Department of Housing and Urban <br /> Development as a participating jurisdiction for the receipt and use of funds as provided by the HOME <br /> Investment Partnerships Program ("HOME Program") as provided in 24 CFR Part 92; and <br /> B. WHEREAS, the County has agreed to use the HOME funds to finance a Homeowner program that meets <br /> the requirement as set forth in 24 CFR Part 92 as amendecf by the United States Housing and Urban <br /> Development Department(HUD). <br /> C. WHEREAS, the Contractor has agreed to rehabilitate the follawing property: <br /> 21�St. ZeQhvrhills. FL 33542 <br /> NOW, THEREFORE, the County and the Contractor do mutually agree as follows: <br /> {1) Housing Quality Standards. Once rehabilitation is completed, the property will meet the property <br /> standards (HQS} as outlined in 24 CFR Part 92.251 and the County will ensure that the property will meet the <br /> requirements of its written standards, applicable local codes, rehabilitation standards, ordinances and zoning <br /> ordinances. Homes that are non-conforming but have been a(lowed to continue by the County or applicab(e <br /> city shall be aflowed to participate in the program. <br /> (2) Rehabllitation Work. The work that will be done on the house shall be in accorclance with the proposal, <br /> attached as an exhibit to this agreement. Additional work shall be approved by using a change order fonn. <br /> Change order forms must be signed by the contractor and the Homeowner, and approved by the County. <br /> It is estirnated that the work will be completed according to the following schedule: <br /> Confract Signing: November 21. 2013 <br /> 50°/a Completion 03/05/14 <br /> 100% Compietion 05/19/14 <br /> The budget for this project is estimated as fallows: <br /> Titie Costs 500.00 <br /> Rehab Work $120.900.00 <br /> Total $121,400.00 <br /> The original contract calls for the work to be completed in 150 days. <br /> (3) Other Program Requirements.The Contractor and the County must comply with all applicable federal <br /> laws and regulations as described in the HUD guidelines (24 CFR Part 92}. <br /> a. Equal Opportunity and Fair Housing. No person in the United State shall on the grounds of race, color, <br /> national origin, religion, handicap or sex be excluded from participation in, be denied the benefits of, or <br /> be subjected to discrimination under any program or activity funded in whale or in part with HOME funds, <br /> This shalf be enforced by Pasco County. <br /> b.
